Sedation Dentistry

Let’s prepare for your procedure.

PREPARATION AND INSTRUCTIONS

Whether your sedation services will take place in the office or at the hospital, please find instructions, forms and more information below.

In-hospital sedation

N

BEFORE YOUR HOSPITAL VISIT

*In-hospital sedation is for special needs patients only.

1. Consultation & Treatment Plan:

Schedule an in-office consultation to discuss your treatment plan. If an in-office evaluation isn’t possible, the evaluation will occur at the hospital before your procedure.

A care plan will be created to address your specific needs, especially for those with high anxiety, health issues, or special needs.

2. Consent Forms & Pre-Op Physical:

Complete the consent packet provided by our team after your hospital treatment time is scheduled. Contact us if you do not receive these forms.

Schedule and complete a pre-op physical with your primary care provider no more than 30 days before your procedure date.

3. Plan Ahead:

Hospital schedules fill up quickly—expect a wait of several months. Plan your visit well in advance and keep regular communication with our hospital coordinator.

AFTER YOUR HOSPITAL VISIT
